For Motor Vehicle Reinstatement/Revocation:

• Verify with ADOT that you are eligible to complete this process

• Arizona Driver License or ID. If you do not have either, know your Driver License Number plus have another form of photo identification  

• Certificates of completion for DUI or Substance Abuse Counseling or Education (if applicable)

• Payment (Cash or Credit Card)

For DUI Screening:

• Arizona Driver License or ID. If you do not have either, know your Driver License Number plus have another form of photo identification.

• Admin Per Se, ticket, or know the exact date of DUI arrest

• Court paperwork (if available)

• Payment (Cash or Credit Card)

For Counseling, Education, or Other Services:

• Photo ID

• Court paperwork (if applicable)

• Payment (Cash or Credit Card)
