Licensed DUI (Driving Under Influence) Service by the Arizona Department of Health Services (ADHS) as well as an authorized service provider of ADOT.
Real Life Therapy Services understands how anxious and intimidating it is to navigate the requirements of the court and motor vehicle systems. We are here to help explain and simplify the process as needed.
Motor Vehicle Reinstatement/Revocation:

This is a requirement of ADOT to be completed if your license has been revoked. This is not applicable if license has been suspended.

Always check with ADOT to ensure you are eligible to begin this process. ADOT will provide you with this packet of information or it may be printed off of the ADOT MVD website.

The packet must be completed by an authorized MVD counselor.

The evaluator may have you complete two substance abuse screening instruments as well as incorporate a face-to-face interview process to determine if “the condition of the applicant does or does not affect his or her ability to safely operate a motor vehicle”.

The Motor Vehicle Division will notify you of your results by mail within 45 days.

Please make sure your address — both mailing and residential addresses — is current with MVD.

DUI Screening/Evaluation:

What do I need to know or bring to my DUI Screening appointment?

Admin Per Se/Ticket (if available): This is the Yellow or Pink copy of the ticket print out provided at the time of arrest. If you have questions, contact the MVD at 1 (800) 251-5866.

Driver License or Arizona ID: If you do not have either of these, you will still need a photo ID and your Driver’s License number. (Your Driver’s License number is located on the Admin Per Se (Ticket).

Violation Date: This information is present on the Admin Per See/Implied Consent or Ticket.

BAC: (if available) This is the Blood Alcohol Concentration at the time of arrest through either the breathalyzer or blood draw.

Court paperwork: (if applicable). The court may request a DUI Screening/Evaluation be completed following the conviction. If this is the case please bring documentation that states what you were convicted of and documents that contain the Docket/Case Number. There are instances where the lawyer may request the screening to be completed prior to court so that you may get your driving privileges back until the court process is complete as well as show the court you are being proactive.
What is a DUI Screening?

A Driving Under Influence screening is a face-to-face interview for approximately 30 to 45 minutes. This determines the next level of services needed.

No Urine Analysis (UA) or blood test required at this visit
-
It is a question/answer process in which one or two written testing instruments are administered (bring reading glasses if you need them).

Addresses the situation leading to the arrest, alcohol concentration at time of arrest, history of alcohol/ drug use, past treatments, possible impairments, as well as obtaining other pertinent information.

Recommendations are based on the testing instruments, interview process, court input and standard ADHS criteria.

Typical recommendations may include: Level I (20 hour DUI Treatment & 16 hours DUI Education), or Level II (16 hours DUI Education). There may be circumstances that required additional services or none at all. This will be discussed at time of service.

All DUI Service results are provided in writing to the court.
A perk to attending the screening with Real Life Therapy Services, is the counselor will help field any questions or provide ongoing informational assistance in navigating the process related to the DUI even if the client chooses to continue services with another provider.

Out-of-State Screenings/Assessments

May require additional time as well as require a multiple step process towards completion based upon that State’s requirements.

Please provide the screener any appropriate documents to be completed and rules surrounding that state’s requirements for the screening or assessment.

DUI Education:
Provided in a classroom setting offered typically on two consecutive 8-hour days addressing the following curriculum:
-
Physiological effects of alcohol and drug use.
-
How an individual’s ability to operate a vehicle is affected by alcohol/drugs; how an Alcohol Concentration level is measured, and how it impacts an individual’s ability to operate a vehicle.

Alternatives to operating a vehicle while under the influences of drugs/alcohol.
-
Psychological and sociological effect of alcohol and drug use.

Stages of substance abuse.
-
Self-assessment of alcohol and drug use.
-
Criminal penalties and statutory requirements for DUI clients.
-
Alternatives to alcohol and drug use.
-
Identification of different approaches to the treatment of substance abuse.
-
Orientation to the process and benefits of group counseling and self-help groups (i.e. AA, NA).
-
Resources, programs, interventions available in the community for substance abuse treatment.
A Pre-test is administered prior to class to measure client knowledge in the above topics. Upon completion of the last class, a Post test is administered to measure amount of information learned.

DUI Treatment:

Substance abuse counseling provided in a group setting with the duration no less than 90 minutes

A minimum total of 20 hours
-
Participation in a self- help group is not considered a replacement for DUI Treatment

The referral agency will be provided monthly reports of the members progress
-
A serialized Certificate will be given to client and referral source at time of completion.

Domestic Violence/Disorderly Conduct Treatment:
​
The program helps the individual to address accountability for their actions, examine beliefs used to justify violence or intimidating anger outbursts, communication in a respectful manner, control and dominance, the role of substance abuse, etc . The general expectation of all participants is to initially promote a significant and long lasting attitudinal change that will reflect into their current and future relationships.

A client is referred for treatment through the Courts, Probation, or Parole for Disorderly Conduct, Domestic Violence, or Assault.

An intake and assessment is completed to get to know the client and their specific needs plus sign all appropriate documents for treatment.

Number of sessions are determined by the court system or combined collaboration between the court and counselor.

Sometimes a client may need to do additional services such as a substance abuse evaluation, substance abuse classes or even parenting.

The client is assigned to group and/or individual therapy based on need.
-
The referral source is informed that the client has begun, been terminated, or completed the program.

Monthly, the referral sources receive a report on progress and attendance​
